      Abstract Submission Instructions
      For successful submission, please follow these four steps:
      1. Prepare your abstract file as described below.
      2. Go to the Abstract Submission Form (click link at bottom of page).
      3. Complete the Abstract Submission Form.
      4. Submit the Abstract Submission Form.

      How to Prepare Your Abstract File
      The following format, organization and size/type of file are required:

      Font, Margins, Layout

      1. Times New Roman 12-point font
      2. 1-inch margins (top, bottom, left, right)
      3. Left-justify all text
      4. Single-space all text except where blank lines are indicated below

      Organization

      1. Title of abstract followed by 1 blank line
      2. List of authors followed by 1 blank line

        Arrow Pointing Right - icon Include for each author: full name, highest degree, and institutional affiliation.

      EXAMPLE:
      Daniel G Smith, PhD, University Medical Center, Anywhere, USA; next author's full name, highest degree, affiliation, etc.

      NOTE:

      1. Presenter's name must be in bold type.
      2. Affiliation (name of institution, city, country) may be stated once per group of authors from the same institution.
      1. Describe Purpose, Methods, Results, and Conclusions -- and define all abbreviations -- followed by 1 blank line
      2. Provide three to five keywords describing the nature and content of the abstract

      Size, Type of File

      1. When printed, entire abstract must fit on one 8.5 x 11-inch page
      2. Save your abstract as a Microsoft Word (.doc) file
