For all other registrants: (presenters
not accompanied by a preceptor, students, preceptors, representatives
from sponsoring company, etc.) You are responsible for making your
own hotel arrangements. The Crowne Plaza has rooms available at
a rate of $89.00 plus hotel and city taxes per night. Call 713-797-110
for reservations and indicate that you are with the conference when
calling for accommodations. |

Traveling to the Conference: |
Travel note: There are two airports
in Houston, William P. Hobby Airport and George Bush Intercontinental
Airport. Hobby airport is closer and more accessible to the medical
center and Conference location. We recommend using Hobby airport
when feasible. |
For presenters accompanied by a preceptor and
traveling more than 300 miles: Navigant International, 1-800-666-5555
will arrange all airline reservations. All flights will required
payment by personal credit card at the time of booking. The Midwest
Pharmacy Residents Conference will reimburse all flights, up to
a maximum of $300 per ticket after conference attendance. Please
call Navigant International prior to March 30, 2001 make flight
arrangements. Airline reservations not made by this agency will
not be reimbursed by the conference. Airline reservations made
after March 20, 2001 may not be reimbursed and will require conference
approval.
For presenters traveling by private vehicle, the conference will
reimburse $0.30 per mile.
Forms for travel reimbursement will be distributed at the conference. |
Ground Transportation:
All conference attendees are responsible for airport to hotel transportation.
Taxicabs are available at an estimated cost of $35-40 to or from
William P. Hobby Airport and $40-45 to or from George Bush Intercontinental
Airport.
Express Shuttle USA operates a shuttle bus between George Bush Intercontinental
Airport and the Crowne Plaza Medical Center at a cost of $20.00
on way, $36.00 round trip and between William P. Hobby Airport and
the Crowne Plaza at a cost of $15.00 one way, $26.00 round trip.

For all other registrants: You are responsible
for making your own travel arrangements. However, Navigant Travel.,
will be happy to assist you in making reservations for you. There
is NO travel reimbursement from the Midwest Pharmacy Residents
Conference for anyone other than presenting Residents/Fellows. |

Audio/video equipment for presenters: |
LCD projectors, IBM-compatible lap top computers
with Powerpoint 97®, slide projectors and laser pointers
will be available for use. MacIntocsh/Apple systems and Web access
will not be available. In order for us to insure that there will
be an adequate amount of equipment available, we need you to tell
us what type of audio/visual equipment you will be using. Please
indicate below: |
